Good Faith Evaluations (GFE)- Update

We implemented Good Faith Evaluations (GFE) as of April 14, 2025 as part of our commitment to patient safety and staying ahead of industry standards. As the aesthetic industry evolves, these protocols are being adopted in more states, and we want to ensure our patients continue receiving the highest level of care.

GFEs are brief evaluations that take less than 10 minutes and can be completed in our clinic or from the comfort of your home via telehealth. These evaluations are required annually for existing patients and for all new patients receiving injectable treatments, including:
💉Botox, Dysport, Jeuveau
💉Dermal Fillers
💉Sculptra

In rare cases, such as significant medical changes or updates to your treatment plan, a follow-up GFE may be requested.

The cost for this evaluation is $50.

However, to help offset this cost, our Medical Director has approved an alternative:

Patients may now submit a letter from their primary care provider stating they are medically appropriate for aesthetic treatments.

✔ Must be dated within the past 12 months

✔ Must be submitted at or before your treatment appointment

If a letter is not provided, a GFE will be required at the time of your visit.

Please note, while GFEs are not yet mandated by the State of Wisconsin, we are proactively adopting this process based on emerging guidance from nursing and medical boards. Our goal is to stay compliant, prioritize your safety, and ensure uninterrupted access to your favorite treatments.
